To request Direct Online Teller Access, simply print this form, complete and mail to:

As soon as your authorization code is activated, you will receive a Users Guide, and Telephone Teller Terms & Conditions.

To make transactions with Telephone Teller you enter your account number and a Personal Identification number (PIN) at the start of each call.

Your authorization code protects your accounts from unauthorized access. Because you choose your authorization code, it is both confidential and easy to remember.

To create your Personal Identification number (PIN), list any four numbers in the spaces on the request form.  Your account will be ready for use in approximately two (2) days after we receive this request.
